With all due respect to everyone here to me this is a case of just because you possibly can, doesn't mean you EVER should.
There's a reason these things are fired from short barrels. The flares we're talking about burn at 3000°f and are intended to leave the host barrel immediately. Even the slightest bit of fouling or other obstruction in your shotgun barrel could cause the flare to stick and you'd end up with a pyrotechnic squib in your barrel burning at 3000°f for up to ten seconds.
The best and most responsible advice I can offer is to call the manufacturers of any flares you intend to use directly and ask if they recommend using their products in a shotgun. Then, call the manufacturer of the shotgun you intend to use and ask if they recommend shooting 12ga flares out of their weapon.
If you don't get the green light from all parties involved, it's just not worth the potential risk to yourself or anyone around you...
